Our telehealth medical weight loss program offers you a convenient and effective way to embark on a journey toward a healthier you. We offer convenience with our telehealth weigh loss option and you can receive expert advice without having to travel for office visits or to the pharmacy to pick up medications. Our medical professionals understand that weight loss is not a one-size-fits-all journey and will work with you based on your medical history, goals and unique needs.

How Do I Get Started?

• Download & complete the packets on the righthand side under “Forms & Links”
• Once completed, scan and email them back to our office manager, Jetta Sargent at jsargent@itiswellde.com OR fax them to us at 302-678-9310 along with a copy of your photo ID and insurance card
• When your completed forms have been received, a member of our office staff will call you to schedule you for your first telehealth visit with our provider and make sure that your payment has been received for the membership using the link below.
• We walk you through the steps the rest of the way!

What Does The Program Include?

• Review of your patient information packet (must be completed and returned) to determine eligibility by our providers.
• Order for comprehensive lab testing
• Cost of medication: monthly supply of Semaglutide OR Tirzepatide injection kit shipped directly to you.
• Monthly telehealth appointments with your provider
• Virtual nurse visit for injection instructions

NOT Included In The Program

• Cost of labs (covered by most PPO insurances)

Price & Membership Details

• $550.00 per month with a signed contract for a minimum of THREE (3) months.
• Payment in full is required before patients can be scheduled for appointments.
• Consultations are available upon request for a deposit of $50. Consultations fees are non-refundable and are not included in the cost of the program.

How do I cancel my subscription?

To cancel your subscription please email JSargent@itiswellde.com or contact our office at 302-678-9355. Please note we have a 3-month minimum for subscriptions.
