Are you a new patient to the It Is Well Healthcare family? We can’t wait to provide you with quality medical services! Please help us save you time during your first visit by completing the new patient packet below. You can fill in the blanks online and print it or download it. You may drop it off at our office, email it to frontdesk@itiswellde.com, or fax it to 302-678-9310. Once we receive your completed New Patient Packet we will promptly call you to schedule an appointment!

Medical Record Release Form
If you are transferring to our practice, you will need to complete a Medical Record Release Form so we can send the request to your former provider.

DIAA Sports Physical Form
If your school-aged child is playing sports, they will typically need a sports physical per the Delaware Interscholastic Athletic Association (DIAA). Your child can receive this form from their school or download it here.
Medical Marijuana Application
In order to qualify for a Medical Marijuana card, you must meet the following qualifications:
Be 18 years of age or older or be a pediatric patient with parental consent.
Be diagnosed with a qualifying medical condition – list can be found here.
Have a Health Care Practitioner Certification from a Delaware licensed MD, DO, APRN or PA.
Be a Delaware resident with proof of residency. (DE Driver’s License or DE State ID)
