What are your office hours?

For your convenience, patients are seen during the following hours:
Monday-Thursday 8:30 a.m.-5:00 p.m.
Friday 8:30 a.m.-12:30 p.m.

The office will be closed daily from 12:00 p.m.-1:00 p.m. for lunch

Phones are answered by office staff during the following hours:
Monday-Thursday 8:30 a.m.-5:00 p.m. (Officer closed daily from 12:00 p.m.-1:00 p.m. for lunch)
Friday 8:30 a.m.-12:30 p.m.

How do I make an appointment?
Call our office at (302)-678-WELL (9355). We do request that you obtain and complete a new patient packet from our office or our website and bring it with you to your appointment. The new patient packet can also be faxed. Click Here for New Patient Packet

Will I be charged for missing an appointment?
Unfortunately, we have to charge a $25 fee for missed appointments when proper notice is not provided. Following four no-shows, you will be dismissed as a client from the practice.

What if I am late for my appointment?
We kindly ask that you arrive on-time for your appointments. We understand life happens to all of us, so we do allow a 15 minute grace period for appointments as long as our schedule allows. If you are more than 15 minutes late, we will need to reschedule your visit.

What if I need forms completed?
1: Blank forms will not be accepted. Personal information must be completed.
2: Please drop off forms 7 days before needed as it may take up to 7 days to complete the documents.
3: Many forms require a current examination prior to being completed.
4: The charge for review and completion of medical forms is $20.00. Payment is due upon pickup. There is no additional fee when forms are brought to a scheduled visit at the office.
5: $10 fee is required for letters written by the provider

How do I transfer my medical records to/from It Is Well Healthcare?
If you are joining our practice, medical record release forms are available to download on our website. Please print the form and drop it off at the office or fax it to 302-678-9310. We will then obtain your medical records from your previous physician.

If you are transferring to a new provider from It Is Well Healthcare, we have a copy service that copies records on a weekly basis. There is a fee to obtain medical records. Please call the office at 302-678-WELL (9355) for more information and pricing details.

Prescription Refills:
Prescriptions that cannot be phoned in may require 48 hours to be completed. Please plan at least 5 days in advance of your medication running out.
Prescriptions will only be refilled during normal office hours.

What do I need to bring to my appointment?
We ask that you bring the following items/documents to your appointment:
Driver’s License or Photo ID
Insurance Card (if applicable)
Payment for Services or Co-Pay
List of Current Medications
List of Past Surgeries and Medical Conditions
List of Any Allergies
Completed New Patient Packet (if you are a new client)
